How to report a Claim:

1. Choose the applicable loss notice for the type of claim:
   
   Adobe PDF Property Loss Notice (for your use)
   Adobe PDF General Liability Loss Notice (for your use)
   

2. Once the loss notice is complete, the claim may be reported directly to the carrier CLICK HERE to
   see a list of TAPCO's carriers and their Claims Reporting phone numbers and email addresses.

3. The claim may be reprted directly to TAPCO via email to claims@gotapco.com (or by fax to 336-538-0094). In addition, you may call 800-334-5579 (dial option 3, then dial option 4).
